Conduct psychosocial assessments, mental health status exams, psychiatric and psychological evaluations to determine behavioral health care plans for patients
Provide evidence-based trauma-informed behavioral health treatment for patients
Implement psychoeducation for patients during individual or group visits
Assist in the construction of clinical pathways, group therapy, classesandbehavior-focused practice protocols
Expedite and supervise referrals to specialty behavioral health services and support a smooth transition from special behavioral health services to primary care
Provide supervision to a smalllocalteam
Serve as a consultant to the interdisciplinaryteam
Participate ininterdisciplinaryteam meetings, providing clinical assessments and measures to coordinate treatment considerations
Assist with the capturing program evaluation and fidelity measures
Engage in on-call crisis line management
Travel could exceed 50 percent of the time and include occasional overnights
Unrestricted Licensed Professional Counselor License, Licensed Clinical Social Worker, Licensed Marriage and Family Counselor License or license eligible
Comprehension of the principles and practices of behavioral health treatment
Skill and expertise in conducting group and individual psychoeducational classes andtherapystrategies
2+ years of integrated behavioral health experience in a primary care setting
2+ years of clinical experience
1+ years management/supervision
Reliable transportation and valid driver’s license
Basic computer proficiency; must be able to type and navigate a Cloud-based environment and designated teleconferencing application
Medicare or Medicaid
A background in maternal/substance abuse populations
